This 48 year old patient presented to my office with a long history of wearing a hair piece. Years earlier he chose to use a hair piece so as to maintain a low hairline with high density and full scalp coverage. He wanted to continue using his hair piece, but was concerned about the appearance of his frontal hairline. He requested that the frontal hairline be restored in a way that would allow him to use his hair piece to maintain high density, full scalp coverage behind the frontal region. The patient wanted to maintain a discrete appearance during the recovery phase. He was asked to grow his frontal hair in the proposed region of the hairline restoration and use the hair piece to maintain scalp coverage behind this zone. A total of 2,689 grafts were placed in the frontal hairline region using high-density grafting and a no-shave technique. The back edge of the graft zone was designed with a sharp edge to allow meticulous abutment of the hair piece to the graft zone. The plan was developed with the possibility of him continuing hair restoration behind the newly grafted hairline at some future date. Future grafting would allow him the option of using a smaller hair piece over the crown or possibly dispensing with the hair piece entirely. Presented here are one year postoperative photos. Comb-back views and hair-part views demonstrate the density match between the graft zone and his current hair piece. The hair-part views also show the tight integration between his hair piece and the grafted region. He was satisfied with the frontal restoration and expressed a desire to continue moving the graft zone back.
